Q I am a 35-year-old housewife who takes Pilates classes. I heard that Chuna manual therapy is good for correcting forward head posture, but is stretching or exercise alone insufficient? I am curious about the difference between Chuna and other methods.

A Exercise and stretching are active management methods for building muscle strength and flexibility, while Chuna is a manual therapy that directly corrects the alignment of misaligned cervical and thoracic vertebrae and improves joint mobility. Since the effectiveness of exercise is limited when joints are misaligned, combining the two yields the best results.

Detailed Answer

Forward head posture is not merely a neck issue; it is an alignment problem involving hyperextension of the upper cervical spine, flexion of the lower cervical spine, increased scapular kyphosis, and shoulder rounding. While stretching and exercise are essential for lengthening shortened muscles and strengthening weakened deep flexor muscles, alignment may not be restored through exercise alone if joint mobility is restricted or segments are misaligned. Chuna manual therapy is a treatment in which a Korean medicine doctor evaluates and directly corrects displacement and limited range of motion in the cervical and scapular segments; it is also covered by automobile insurance and health insurance (up to 20 times per year). In Korean medicine, kidney function governs bones, while liver function governs tendons. This is why skeletal alignment (Chuna) and myofascial tension (acupuncture and moxibustion) must be addressed together. If the flow of Qi and blood along the neck and back lines is blocked due to forward head posture, circulatory stagnation in the surrounding muscles will cause the alignment to collapse again even after correction. Therefore, in clinical practice, the treatment is structured in stages: restoring alignment through Chuna manual therapy, loosening stiff fascia with acupuncture and moxibustion, and reinforcing support in segments with weakened ligaments using thread embedding and fire acupuncture. When active exercises like Pilates are added to this, the body develops the ability to maintain the corrected alignment on its own. For those whose muscles have lost nutrients due to accumulated fatigue, replenishing Qi and blood with herbal medicine tailored to their constitution is necessary for the corrective effects to last longer.
