A Aerobic exercise is one of the non-pharmacological treatments for fibromyalgia with the highest level of evidence. The key is to start with low-impact aerobic exercises, such as aquatic exercise (aqua aerobics), walking, and cycling, at a low intensity and gradually increase it. You need the flexibility to start with 5 to 10 minutes and reduce the amount if pain worsens.

Exercise acts in fibromyalgia through various mechanisms, including promoting endogenous opioid secretion, alleviating central sensitization, improving sleep quality, and reducing depression and anxiety. The exercises with the highest level of evidence are aquatic aerobic exercise (heated pools) and walking. Resistance training (strength training) is also effective in reducing pain, but lifting heavy weights from the start can worsen symptoms. Studies also suggest that Tai Chi and yoga are beneficial for improving flexibility and reducing stress. The key principle is to maintain an intensity where pain does not worsen after one day compared to before starting. In Korean traditional medicine, appropriate exercise is recommended based on the principle that "movement facilitates the circulation of Qi and blood." However, since overwork depletes the body's fundamental energy, gentle movements that gradually circulate Qi and blood are more suitable than vigorous exercise for those who lack internal fluids and tire easily.
