#Causes Explained

Q I am an office worker in my 30s who runs every weekend, but I keep experiencing pain in the front of my hip and in my groin area. Is this caused by exercise?

A In young adults, pain in the front of the hip and groin following repetitive impact exercises such as running, cycling, or soccer is often caused by iliopsoas tendinitis, hip impingement syndrome, or labral tears. Lack of flexibility or muscle strength imbalances in the muscles surrounding the pelvis and hip joint also contribute. Early evaluation is important because neglecting these conditions can lead to cartilage damage.

Detailed Answer

The primary cause of exercise-related pain in the front of the hip is iliopsoas tendinitis, where micro-damage accumulates in the tendon due to repeated flexion movements. A labral tear involves damage to the cartilage of the acetabulum rim and is characterized by a 'clicking' sound when walking or deep groin pain. Femoral nerve entrapment or iliotibial band syndrome can also cause similar symptoms. It is recommended to check for structural damage using MRI or ultrasound examinations. Korean Medicine Clinic Perspective In Korean medicine, musculoskeletal damage caused by excessive exercise is viewed as a condition where pain arises due to blocked Qi and blood circulation. The meridians passing through the inner groin are connected to organs responsible for digestion and absorption; therefore, if this energy weakens, muscles fatigue rapidly and recovery slows down. If blood stasis accumulates around the psoas muscle and hip ligaments due to repetitive exercise, blood circulation is blocked, leading to persistent pain. At Dongjedang, we reduce inflammation in the tendinous area using ultrasound-guided pharmacopuncture, restore normal range of motion by releasing ligament adhesions with needle acupuncture, and aid tissue regeneration by removing internal blood stasis through internal treatment.
