Q I am a housewife in my 50s, and the area below the inner side of my knee keeps hurting when going up and down stairs. What causes pes anserinus tendinitis?

A Pes anserinus tendinitis is a condition in which inflammation occurs in the bursa due to accumulated overload at the common attachment site of the sartorius, gracilis, and semitendinosus muscles located beneath the inner side of the knee. It is prone to occur if one frequently uses stairs or repeatedly adopts a posture where the knee leans inward. Overweight, degenerative changes in the knee, and flat feet are also major risk factors.

Pes anserine refers to the area where the sartorius, gracilis, and semitendinosus muscles attach jointly to the medial side of the tibia. Repetitive irritation to the bursa located beneath this area leads to inflammation. It is frequently associated with degenerative knee osteoarthritis, and structural factors such as obesity, diabetes, and flat feet accelerate its onset. It commonly occurs in housewives who frequently use stairs, individuals who walk uphill often, and runners. In Korean traditional medicine, based on the principle that the energies of the spleen and kidneys govern muscles and bones, it is believed that if these functions weaken, the tissues surrounding the muscles and bones do not receive proper nutrition, leading to the stagnation of dampness and blood stasis. The stagnation of qi and blood in the meridians passing through the medial side of the knee is directly linked to pain in the pes anserine area.
