Q Can carpal tunnel syndrome or trigger finger occur at the same time as wrist tenosynovitis?

A Yes, there is a high likelihood of co-occurrence. Since wrist overload is a common cause, it is not uncommon for carpal tunnel syndrome (median nerve compression) and trigger finger (flexor tendon nodules) to occur together. As these are lesions in different areas, it is important to accurately differentiate the diagnoses to establish a treatment plan.

Detailed Answer

Although wrist tenosynovitis (compartment 1 tendon sheath), carpal tunnel syndrome (median nerve compression within the carpal tunnel), and trigger finger (flexor tendon nodules) differ in their sites of occurrence and mechanisms, they share common risk factors such as repetitive hand use, diabetes, thyroid disease, and pregnancy, resulting in a high co-occurrence rate. In particular, diabetic patients have a 2 to 3 times higher incidence rate of all three diseases compared to the general population due to the accumulation of AGEs (glycated products) in tendon tissue. During diagnosis, we simultaneously check the Tinel test (carpal tunnel), Finkelstein test (tenosynovitis), and the presence of trigger points.

Korean Medicine Clinic Perspective

All three of these conditions are deformities in which fluid, waste products, and stagnant blood accumulate in the tendons of the hand, blocking circulation. When digestive and absorption functions decline and fluid and waste products accumulate, these conditions can manifest through various pathways, such as the carpal tunnel, tendon sheath, and tendon nodules. Dongjedang adopts an integrated approach that distinguishes associated conditions and applies different acupuncture sites appropriate for each area, while using personalized herbal medicine to simultaneously resolve the common causes of fluid, waste products, and stagnant blood.
