#Treatment Stages

Q In what order is treatment performed during the early (acute) phase of costochondritis? I would like to know what is done when I first visit the Korean medicine clinic.

A During the first visit, the painful area is palpated, the state of inflammation is checked via ultrasound, and internal medical causes, such as heart conditions, are differentiated. Subsequently, tension in the surrounding muscles and fascia is relieved with acupuncture, and local anti-inflammatory treatment is administered using pharmacopuncture or bee venom acupuncture. During the acute phase, rest is prioritized over heat therapy, often combined with cold compresses. The decision to use moxibustion is made after observing the progress following 2 to 3 treatments.

Detailed Answer

The initial treatment stages for costochondritis proceed as follows: ① Confirmation of Diagnosis: Palpation of tender points at the costochondroid junction + confirmation of inflammation and swelling around the cartilage via ultrasound. If necessary, a consultation with an internal medicine specialist is recommended to rule out heart-related symptoms. ② 1-2 Treatment Sessions: Relief of defensive tension in the intercostal muscles with acupuncture, and alleviation of acute inflammation with pharmacopuncture (anti-inflammatory components). ③ 3–5 Treatment Sessions: After confirming the degree of pain relief, detach the adhesions around the cartilage using a needle. ④ Acute Phase Self-Care: Apply an ice pack for 15 minutes three times (24–48 hours for the acute phase), then switch to heat therapy. Avoid pain-inducing postures during daily activities.

Korean Medicine Clinic Perspective

The acute phase is the initial stage where Qi and blood have just begun to stagnate; the key is to rapidly facilitate circulation before the blockage hardens. Acupuncture is used to stimulate the thoracic points responsible for Qi and blood circulation to open the flow of Qi, while pharmacopuncture is used to cool local heat and toxins. If treated sufficiently during this period, stagnant blood is resolved before it settles in the small circulatory pathways, preventing the condition from becoming chronic.
