#Causes Explained

Q I am a housewife in my 50s, and a bulge resembling a lump has formed behind my knee. Why do Baker's cysts occur?

A A Baker's cyst is a condition in which joint fluid accumulates in the popliteal fossa behind the knee joint, causing it to swell up like a sac. It primarily occurs when joint fluid is excessively secreted due to conditions such as knee arthritis, meniscal tears, or synovitis. When internal joint pressure increases, fluid is pushed out toward the weaker area, the popliteal fossa, forming a cyst.

Detailed Answer

A Baker's cyst, also known as a popliteal cyst, is a cystic structure formed in the popliteal fossa behind the knee where the synovial membrane stretches and joint fluid accumulates. Knee arthritis, meniscal tears, rheumatoid arthritis, and synovitis are the main causes, where excessive inflammatory exudate within the joint forms an outlet toward the popliteal fossa. In adults, it mostly occurs secondarily, and the key is to treat the underlying joint lesion rather than the cyst itself.

Korean Medicine Clinic Perspective

In Korean medicine, the formation of fluid-filled cysts in the popliteal region is viewed as the result of waste fluids and sticky body fluids (phlegm) accumulating between muscles and joints. When the functions governing digestion and metabolism weaken, fluid metabolism becomes inefficient, causing damp waste products to clump together in one area. Such cysts are prone to forming in the popliteal region when the Qi and blood pathways running through the inside of the knee become blocked and stagnant. At Dongjedang, we approach this by releasing adhesions around the cyst using acupuncture needles and special flat-tipped needles, reducing local inflammation with pharmacopuncture, and addressing the root cause of fluid metabolism with herbal medicine that restores digestive and metabolic functions.
