Q I would like to know the order in which Dongjedang treats adenomyosis.
A The process proceeds in the following order: ① Diagnosis assessment → ② Restoration of uterine temperature using moxibustion and deep tissue heat therapy → ③ Dispersion of blood stasis in the muscle layer using blood-activating and blood-resolving herbal prescriptions such as Gyejibokryeonghwan and Dohongsamultang → ④ Recovery of bleeding loss through blood-replenishing and nourishing treatments → ⑤ Lifestyle management to prevent recurrence.
Detailed Answer
During the initial consultation, the depth of blood stasis and the state of coldness in the uterus are assessed based on pain patterns, menstrual flow, presence of anemia, and OB/GYN test results. Subsequently, moxibustion and deep tissue heat therapy are used to raise the temperature of the uterine muscle layer to initiate the circulation of hardened blood stasis. The second step involves dispersing the blood stasis that has invaded the muscle layer using herbal prescriptions that resolve lower abdominal blood stasis or prescriptions that activate blood-activating and blood-resolving herbal prescriptions to clear blocked blood and provide nourishment. If there is blood loss due to excessive bleeding, blood-replenishing prescriptions are administered concurrently to address anemia as well. Once pain and bleeding stabilize, we switch to a nourishing prescription designed to constitutionally improve the cooled uterine environment.
Korean Medicine Clinic Perspective
Since blood stasis in adenomyosis is located in deeper layers than in endometriosis, the treatment period may be longer. We proceed step-by-step through a four-stage process of warming the body, releasing stagnant blood, replenishing lost blood, and strengthening the foundation. Rather than expecting quick results, the Dongjedang approach involves consistently continuing the treatment while monitoring gradual improvement with every menstrual cycle.
