Q When I get up in the morning or turn my head, the ceiling suddenly spins fast. Why does this dizziness come on so suddenly?

A When the otoliths (calcium carbonate crystals) that are attached to the balance organ in your inner ear break loose and drift into the semicircular canals, the lymph fluid sloshes excessively each time you move your head, producing strong spinning vertigo that lasts tens of seconds. A key feature is that it appears only with changes in position, without any hearing problems.

Detailed Answer

The vestibular organ deep in the ear contains otoliths (calcium carbonate crystals) that sense position and acceleration. When these leave their normal place and enter the semicircular canals, which detect rotation, BPPV (Benign Paroxysmal Positional Vertigo) develops. When you turn your head or lie down and get up, the displaced otoliths disturb the flow of the endolymph, causing the brain to receive a false rotation signal; this triggers intense vertigo that usually settles on its own within a minute and tends to recur. A hallmark of BPPV is that it is not accompanied by hearing loss or ear fullness, which distinguishes it from Meniere's disease or vestibular neuritis. Diagnosis is made by confirming the characteristic nystagmus (eye twitching) with the Dix-Hallpike test.
