BPPV typically presents as brief spinning vertigo that occurs only with changes in position, without any hearing problems. By contrast, if you have any one of the following, you should strongly suspect central dizziness with the brain as the cause, such as a cerebellar or brainstem stroke: (1) weakness or sensory changes in one arm or leg, (2) slurred speech or inability to speak, (3) double vision in which objects appear as two, (4) the worst headache of your life, or (5) such severe unsteadiness that you cannot walk. Immediate emergency services or an emergency room visit is required.
